Art and Change Grant Reporting Requirements

All 2012 grantees must submit a final report to Leeway within 60 days of completing your project.

Please notify Leeway of any changes to your project.

If you have an outstanding report, you will not be able to apply for another Art and Change Grant or Transformation Award.

Transformation Award Reporting Requirements

All 2012 awardees must submit a final report to Leeway by March 15, 2014.

Any audio or visual documentation of your work is welcomed for promotional use but is not required.

If you have an outstanding report, you will not be able to apply for an Art and Change Grant or another Transformation Award.
