Art and Change Grant (ACG) Reporting Requirements

All grantees must submit a final report to Leeway within 60 days of completing your project.

Please notify Leeway of any changes to your project.

If you have an outstanding report, you will not be able to apply for another Art and Change Grant or Transformation Award.

Leeway Transformation Award (LTA) Reporting Requirements

All 2016 awardees must submit a final report to Leeway by March 15, 2018.

Any audio or visual documentation of your work is welcomed for promotional use but is not required.

If you have an outstanding report, you will not be able to apply for an Art and Change Grant or another Transformation Award.

Window of Opportunity (WOO) Grant Reporting Requirements

A final report is due 90 days after the opportunity end date.

Retain all receipts associated with your opportunity request. You will be required to submit copies of receipts associated with your opportunity with your final report.

Any funds not spent for the purpose specified in the application must be returned with your final report. Please contact program staff with any questions about this.
